How to fill out Form 5472 and the pro forma 1120, explained simplyCómo llenar el Formulario 5472 y el 1120 pro forma, explicado fácil

Two documents, seven parts, and a lot of fear for nothing. This is what each part asks for, in plain language.

Short answer

A foreign-owned single-member LLC sends two documents together: a pro forma Form 1120, with only a few boxes filled in, and Form 5472 attached to it. Form 5472 asks who the LLC is, who owns it, and how much money moved between the LLC and its owner during the year. Then you fax or mail both to the IRS.

Before you start: what to gather

  • The LLC's EIN, legal name, address and the date it was formed.
  • What the business does, in a few words.
  • The owner's full name, address, country of citizenship and country of residence.
  • The owner's tax ID from their home country (the IRS calls it FTIN). If you do not have one, you will write "None".
  • Bank statements for the whole year, plus receipts for company expenses you paid from your personal account.

Document 1: the pro forma Form 1120

This is not a full corporate return. According to the IRS, the only information required is the LLC's name and address and items B and E on the first page: the EIN, and the boxes that say whether this is a first return, a final return, or a name or address change. Write "Foreign-owned U.S. DE" across the top. No income or tax is calculated on it.

Document 2: Form 5472, part by part

PartWhat it asks, in plain words
Part IYour LLC: name, address, EIN, total assets and main business activity.
Part IIYou, the foreign owner: name, address, countries of citizenship and residence, and your FTIN (or "None").
Part IIIThe "related party" the LLC did business with. Usually it is you again, and the IRS asks you to complete it even if you already appear in Part II.
Part IVMoney paid or received with the foreign owner for things like sales, services, rent, loans or interest.
Part VA box to check if the LLC had other transactions with you, such as money you put in (contributions) or money you took out (distributions), or amounts for forming or closing the LLC. You describe them on a separate attached statement.
Part VIExchanges with no money, or for less than full value (for example, using your laptop or services without payment).
Part VIIA few yes/no questions that every filer must answer.

A simple example

Laura lives in Colombia and owns a Florida LLC. In 2026, she transferred $8,000 from her personal account to open the business, the LLC paid her back $3,000, and she paid the $138.75 Florida annual report from her own card. All of these are transactions between Laura and her LLC. Laura checks the Part V box and attaches a short statement listing each type of transaction and its total in U.S. dollars. If she had also loaned money to the LLC, or charged it for services, those amounts would go in Part IV.

Rules that make it easier

  • Everything goes in U.S. dollars. If you used another currency, attach a schedule showing the exchange rates you used.
  • "$50,000 or less" is allowed. If the total of a type of transaction is $50,000 or less, the instructions let you write "$50,000 or less" instead of the exact amount.
  • Reasonable estimates are accepted when the exact amount cannot be determined: the IRS defines them as between 75% and 125% of the real amount.

How to send it

  1. Put the pro forma Form 1120 on top and Form 5472 (with its statements) behind it.
  2. Fax everything to 855-887-7737 (300 DPI or higher), or mail it to: Internal Revenue Service, 1973 Rulon White Blvd, M/S 6112, Attn: PIN Unit, Ogden, UT 84201.
  3. Keep a full copy and the fax confirmation or mailing receipt.

The deadline and the extension are explained in our guide on Form 5472 due dates.

Common mistakes

  • Leaving the FTIN box blank. If you do not have one, write "None" or "N/A".
  • Forgetting the Part V statement for the money you put in or took out.
  • Sending it to the regular Form 1120 address. Foreign-owned LLCs have their own fax number and address.
  • Sending an incomplete form. The IRS treats a substantially incomplete Form 5472 as not filed, and the penalty is $25,000.

Respuesta corta

Si no tienes, escribirás "None".

  • Se permite escribir "$50,000 or less".

Errores comunes

  • Dejar en blanco la casilla del FTIN. Si no tienes, escribe "None" o "N/A".
